TOC Clinical Pharmacist

Clinical Pharmacist Responsibilities: The TOC Clinical Pharmacist shall support the mission of the hospital by aiming to reduce hospital readmissions through patient discharge follow-up. Perform clinical duties and engage in patient care to assist with medication safety, increase patient medication and disease state understanding, and encourage provider follow-up and continuity of care following hospital discharge.
