Remote Nurse (LPN/LVN) - Transforming Healthcare through Technology

We're seeking a compassionate and dedicated LPN/LVN to join our remote team, delivering Remote Therapeutic Monitoring (RTM) and Chronic Care Management (CCM) services that empower patients to take control of their health from the comfort of their own homes.

Job Summary: As a Remote Nurse (LPN/LVN), you will play a vital role in our mission to innovate patient care through technology. You will facilitate patient enrollment, conduct daily check-ins, and provide personalized support to ensure patients receive the best possible care. Responsibilities:

• Enroll patients in our RTM and CCM programs using our state-of-the-art portal
• Conduct daily calls and text messages to monitor patient health status and provide ongoing support
• Collect and document patient health information, including medical histories and new symptoms
• Provide patients with valuable health resources, such as stretches, meditation videos, and light exercises
• Maintain strict confidentiality and adhere to HIPAA regulations and company policies
• Participate in ongoing training and check-ins to ensure you have the tools and expertise needed to excel in your role

• A quiet, dedicated workspace for full-time remote work

Requirements:

• Current LPN certification
• 3+ years of experience in office or remote healthcare settings
• Proficiency with technology, including Zoom, Slack, and online enrollment platforms
• Strong organizational skills and ability to manage multiple tasks efficiently
• Excellent communication skills, both verbal and written, with a focus on empathy and patient support
• Ability to work independently in a remote setting, with initiative and self-motivation
• Familiarity with HIPAA regulations and a commitment to maintaining patient privacy
