Summary
The Clinical Pharmacist is responsible for maintaining pharmaceutical dispensing functions, including clinical review functions, in accordance with all legal and regulatory requirements. The Clinical Pharmacist provides pharmaceutical care and products to all neonate, child, adolescent, adult, and geriatric patients, including inpatients and outpatients. This individuals responsibilities include (but are not limited to): All general responsibilities of a Staff Pharmacist. Provide direct patient-centered care and integrated pharmacy services in collaboration with physicians, nurses, and other healthcare professionals. Routine attendance on medical rounds (for decentralized shifts) with consistent documentation of pharmacist interventions. Deliver medication therapy management and ensure pharmaceutical care programs are appropriately integrated throughout the institution. Assist in development and implementation of facility-specific protocols, guidelines, and reference documents related to medication therapy management.
