Remote Case Manager Registered Nurse | WFH
Position: Registered Nurse Case Manager - Remote (Work From Home) Schedule: Monday to Friday, 8:00 AM - 5:00 PM Location: Remote (Candidates must reside in one of the following counties: Macomb, Wayne, Barry, Van Buren, Kalamazoo, Calhoun, Branch, St. Joseph, Berrien, or Cass). While primarily remote, this role necessitates travel to patient residences for assessments.

Hourly rate: $44.14 Benefits available to full-time employees after 90 days of employment 401(k) plan with company match for full-time employees Job Overview We are in search of a dedicated and knowledgeable Registered Nurse Case Manager to join our esteemed team. This pivotal role involves delivering personalized care coordination and leveraging your clinical acumen to assist members throughout their healthcare journeys, ensuring their health requirements are addressed effectively and efficiently. Key Responsibilities Collaborate with patients to evaluate their needs, formulate care plans, and connect them with appropriate services and resources for optimal health outcomes. Utilize clinical tools and data to conduct comprehensive assessments, review health eligibility, and create tailored care plans that cater to both medical and social needs. Ensure adherence to benefit plans, applicable criteria, and regulatory guidelines while interpreting case management standards. Partner with internal and external stakeholders to deliver a seamless, cost-effective care experience. Follow quality management processes and company policies to provide exceptional care aligned with regulatory and accreditation standards. Required Skills Active Registered Nurse (RN) license with 2+ years of clinical experience. Case Management Certification (CCM) is preferred. Experience in Home Health is advantageous. Proficient in Microsoft Office Suite (Word, Excel, Outlook). Associate's degree or higher in Nursing.
