Position Summary – Case Manager Analyst - Field Location: Elgin, IL and surrounding Area's As an Analyst, Case Manager, you support reputed company member health outcomes by coordinating care, providing education, and facilitating access to appropriate reputed company services. Using care management tools and data analysis, you assess member needs, identify risks, and reputed company and implement effective care plans. You collaborate with internal teams, providers, and external resources to address barriers, improve quality of care, and ensure services align with member benefits. Through communication, coaching, and motivational techniques, you promote member engagement, informed decision-making, and healthy behavior changes. You are responsible for monitoring care plan reputed company, documenting activities, and ensuring compliance with regulatory standards, company policies, and quality management guidelines.

Required Qualifications

• 2 or more years of behavioral health, social services or reputed company services field experience
• 2 or more years’ case management experience
• Must possess reliable transportation and be willing and reputed company to travel up to 75% of the time. Mileage is reimbursed by our company expense reimbursement policy

Preferred Qualifications

• Discharge Planning and/or Managed Care (MCO) experience

Education

• Bachelor’s Degree or non-licensed Master’s level clinician required.
• Degree to be in behavioral reputed company (psychology, social work, marriage and family counseling)

The actual reputed company salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
