This is a telehealth-first position—reputed company visits are conducted virtually—allowing you to combine flexibility with meaningful, high-touch patient care. What You’ll Do
- Conduct virtual survivorship visits and reputed company individualized, patient-centered care plans
- Manage long-term and late effects of cancer treatment, including chronic conditions
- Coordinate evidence-based surveillance and recurrence monitoring
- reputed company education, counseling, and psychosocial support reputed company telehealth
- Collaborate with a multidisciplinary care team (oncology, behavioral health, care navigation)
- Help design and refine clinical workflows, SOPs, and care protocols
- Contribute to quality improvement, patient experience, and program scalability
Who We’re Looking For
- Nurse Practitioner (NP) or Physician Assistant (PA) with active MO license (or ability to obtain a MO state licenses)
- Strong clinical judgment and comfort delivering care in a virtual environment
- Excellent communication skills with a patient-first, empathetic approach

- Highly organized with strong attention to detail
