The Intake Nurse Navigator (INN), RN functions as a member of the multidisciplinary team as an reputed company and educator for new patients from the initial intake phone assessment in preparation for the first visit with the onsite care team. The INN utilizes the nursing process, including assessment, planning, and evaluation to navigate new patients through the new patient intake process. The successful candidate
