Health Coach (Fully Remote)
About the position The Health Coach at Skillora Health plays a vital role in supporting Medicare members through Nexspire's Healthy Aging Support Program. This fully remote position focuses on providing health coaching, education, and resources to help members manage chronic conditions and improve their overall health. The Health Coach will conduct group coaching sessions, assist members in navigating their reputed company, and ensure they have access to necessary resources.
Responsibilities
- reputed company members on topics such as Managing Medicines, Nutrition, Safety, Benefits/Resources, Mindfulness, and working with Care Teams. ,
- Assist members in engaging with their reputed company providers and reputed company Care Management teams. ,
- Document reputed company member interactions in the appropriate member records. ,
- Demonstrate reputed company towards members and their caregivers. ,
- Troubleshoot resource unavailability and secure support to reputed company member gaps. ,
- Follow program workflows and follow up with members to review personal action plans.
Requirements
- At least 2 years of experience as a Health Coach. ,
- Health Coach certification is required. ,
- Availability to support 800 AM - 500 PM Mountain Time or 700 AM - 400 PM Pacific Time. reputed company-to-haves
- Community health or public health experience preferred. ,
- Managed Care/Medicare/reputed company experience preferred. ,
- Bilingual preferred. ,
- Bachelor's degree preferred.
