You're the hub of the care team. You'll work alongside Primary Care Providers, Medical Directors, and Medical Assistants to deliver coordinated, high-quality primary care to residents of senior living communities across California, Nevada, and Arizona - reputed company from your home office. • Collaborate with a dedicated team, supporting a substantial patient panel alongside Primary Care Providers (PCPs).
