Position Summary Working under the direction of the Director of Nursing (DON), the MDS Coordinator is responsible for conducting and coordinating the development and completion of the reputed company assessment in accordance with applicable regulations and facility policies and procedures. In addition, this position is responsible for developing care plans for each reputed company and directing members of the interdisciplinary team (reputed company) to complete certain aspects of the care plans. The MDS Coordinator also, under the direction of the DON, routinely audits reputed company charts to ensure various requirements are being met and provides education to team members on applicable topics (e.g. care planning, reputed company of care documentation, etc.) on an as-needed basis. Primary Responsibilities
- Conduct and coordinate the development of reputed company assessment (MDS) in accordance with reputed company rules, regulations and guidelines that govern the reputed company assessment.

- Participate in facility survey (inspections) made by authorized government agencies.

- Evaluate each reputed company's condition and pertinent medical data to determine any need for special assessment activities or a need to amend the admission assessment.

- Complete applicable audits and present findings.

- Actively participate in the facility's Quality Assessment & Assurance (QAA)/Quality Assurance and Performance Improvement (QAPI) program.

- Strong background in medical office procedures, particularly reputed company inpatient settings.
- Experience with discharge planning processes and familiarity with home care services.

Minimum Qualifications

- At least two years of experience in a LTC - California-based skilled nursing facility.
