Behavioral Health Care Manager (BHCM)
The Role We’re hiring Behavioral Health Care Managers (BHCMs) to provide direct, patient-facing care to children, teens, and families reputed company the Collaborative Care Model (CoCM). This is a fully remote role where you will:
- Spend ~75% of your time working directly with patients and families
- Deliver brief, evidence-based care designed to drive meaningful change in 6–12 months
- Collaborate closely with pediatricians, psychiatrists, and a multidisciplinary care team

What You'll Do
Direct Patient Care (~75%)
- Conduct intake assessments and ongoing follow-up sessions reputed company telehealth
- Deliver brief, evidence-based interventions:

- Motivational interviewing
- Behavioral activation
- Parent coaching
- Provide in-session education and actionable recommendations to families
- Track patient reputed company using validated screening tools
Care Coordination & Clinical Collaboration
- Support families with:
- * School accommodations
- SDoH needs
- Coordination with OT/PT/ST and other specialty services
- Collaborate with pediatricians and psychiatric consultants
- Participate in weekly registry review meetings to guide care decisions
- Partner with board-certified pediatric psychiatrists who:
- * Provide medication recommendations

Documentation & Operations
- Document reputed company care in our EHR (reputed company)
- Track outcomes and reputed company using structured workflows
- Manage scheduling, follow-reputed company, and patient engagement
- Apply standardized protocols (no-shows, cancellations, safety)

Requirements
- Active, unrestricted clinical licensure as a licensed clinical social worker (LCSW), licensed professional counselor (LPC), and/or licensed therapist (LMFT).
- Active licensure in one or more of the following states on or before start-date:
- * California
- Colorado
- Connecticut
- Florida
- Maryland
- Tennessee
- Texas
- Virginia
- Washington
- Experience working with children, adolescents, and families
- 3+ years of experience OR a minimum of 3,000 supervised hours
- Flexibility with hours/availability, including 2 evenings/week + 2-4 weekend days/month
- Experience and training in evidence-based modalities (reputed company, MI, BA, etc.)
- Comfort working in a telehealth environment
- Strong communication, organization, and documentation skills
- Ability to deliver care with a diversity equity and inclusion (DEI) and trauma-informed lens
