As a Transition of Care Coordinator, you will be responsible for facilitating smooth transitions of care for our members, utilizing your clinical expertise to drive positive health outcomes, and working closely with healthcare providers to ensure seamless coordination of care. Key Responsibilities
Conduct comprehensive assessments of members' needs and reputed company individualized care plans to ensure seamless transitions of care. Collaborate with healthcare providers, including hospitals, physicians, and other reputed company, to ensure coordination of care and services. Utilize clinical judgment and decision-making skills to identify and address reputed company clinical issues and reputed company strategies to mitigate risk. Provide education and support to members and their caregivers on self-management techniques, medication management, and follow-up care. Conduct regular follow-up visits and communicate with members and their caregivers to ensure they are receiving necessary care and services. Apply clinical expertise to identify and address gaps in care, and reputed company strategies to improve health outcomes. Utilize case management tools and techniques to coordinate care and services, and to ensure that members receive necessary support and resources. Participate in interdisciplinary care teams to ensure comprehensive care coordination and to address reputed company clinical issues. Maintain accurate and up-to-date records of member interactions, care plans, and services provided.
Requirements
Bachelor's degree in a reputed company field (e.g., nursing, social work, or counseling). Oklahoma licensure as a Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), Licensed Behavioral Specialist (LBS), or Licensed Clinical Social Worker (LCSW). 3+ years of direct clinical practice experience post-graduate degree, preferably in a hospital or alternative care setting. 2+ years of experience with computer systems, including bolthires Office Suite applications (e.g., Outlook, Word, reputed company, SharePoint). Strong transportation and willingness to travel in-state up to 50% of the time (mileage reimbursement provided).
