Remote Licensed Clinical Social Worker (LCSW)
Role Overview Gotham Enterprises Ltd is seeking a dedicated and experienced Licensed Clinical Social Worker (LCSW) to join our compassionate team remotely from Beaumont, Texas. This full-time position offers an opportunity to make a significant impact within our Hospice Center, providing vital support to patients and their families during challenging times.
Key Responsibilities
- Conduct comprehensive assessments of patients and families to identify emotional, social, and environmental factors affecting their health.
- Develop and implement individualized treatment plans that address the unique needs of patients in hospice care.
- Provide counseling and support to patients and families, helping them navigate the complexities of end-of-life care.
- Collaborate with a multidisciplinary team to enhance patient care and improve overall quality of life.
- Maintain accurate and timely documentation of patient interactions and treatment progress.
- Participate in staff meetings and contribute to the continuous improvement of hospice services.
Required Qualifications
- Master’s degree in Social Work from an accredited institution.
- Current Licensed Clinical Social Worker (LCSW) certification.
- Proven experience in hospice or palliative care settings preferred.
- Strong communication and interpersonal skills, with the ability to build trusting relationships with patients and families.
- Proficiency in using electronic health record (EHR) systems.
