Work Environment

This role primarily operates in a remote setting, allowing you to balance professional responsibilities with personal commitments. We utilize a range of advanced technology and electronic health record systems, including our proprietary ChartEasy™, to streamline patient management and documentation. Our supportive work culture emphasizes teamwork, where nurse practitioners and physician assistants are available at all locations to assist with care delivery, ensuring that physicians can focus on patient interactions without added pressure.

Key Responsibilities

  • Serve as the attending physician for residents in skilled nursing facilities, delivering high-quality, patient-centered primary care.
  • Collaborate with a team of Nurse Practitioners (NP) and Physician Assistants (PA) to manage daily resident care effectively.
  • Conduct regular rounds and assessments to diagnose and treat a variety of acute illnesses, chronic diseases, and complex health conditions, including diabetes, hypertension, and heart disease.
  • Utilize electronic health record systems, including ChartEasy™, PointClickCare, MatrixCare, and American HealthTech, for patient documentation and administrative tasks.
  • Engage in Theoria's Chronic Care Management and Transitional Care programs, underpinning our commitment to proactive healthcare management.
  • Supervise and mentor collaborating nurse practitioners and physician assistants, fostering a collaborative team environment.
  • Participate actively in medical quality initiatives aimed at enhancing overall patient care and organizational effectiveness.
  • Ensure efficient communication with the virtual care team who manages call coverage, allowing for better work-life balance.

Patient Population / Client Focus Your primary focus will be on residents within skilled nursing facilities, many of whom may present with complex medical needs. You will foster meaningful connections with these patients, providing them with compassionate care tailored to their unique health situations. Theoria Medical prioritizes continuity of care, allowing you to develop long-lasting relationships with patients and their families as you guide them through their healthcare journeys.

Qualifications

  • Must hold a Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree with board certification in family medicine, internal medicine, or a related specialty.
  • Possess an active and unrestricted medical license in North Carolina.
  • Demonstrable experience in primary care, particularly in managing patients with chronic conditions and acute illnesses.
  • Excellent interpersonal and communication skills, fostering effective collaboration with healthcare team members.

Preferred Experience

  • Experience working within skilled nursing facilities or similar post-acute care environments is highly desirable.
  • Familiarity with electronic health record systems and telemedicine platforms.
  • Previous engagement with chronic care management initiatives or similar quality improvement programs.
