Community Case Management Manager

Responsibilities

  • Manage and oversee implementation of the statewide Adult Community Case Management program serving adults with Intellectual Disabilities and/or Autism, including services funded through MaineCare Section 13 Targeted Case Management, under the direction of OADS leadership.
  • Supervise, coach, and provide day-to-day directions to a statewide team who conduct provider support, monitoring, technical assistance, licensing and certification activities, and issue resolution within the Adult Community Case Management system.
  • Provide operational management and subject matter expertise on MaineCare Section 13, Home and Community-Based Services requirements, conflict-free case management expectations, person-centered planning, and applicable OADS and OMS policies.
  • Oversee initial certification and recertification reviews of Adult Community Case Management provider agencies and coordinate with OADS, the Office of MaineCare Services, and the Division of Licensing and Certification regarding 10-144 C.M.R. Chapter 108, Home and Community Support Services Licensing Rule, including provider qualifications, licensing standards, policies, staffing, supervision, documentation, person-centered planning, quality assurance, training compliance, health and safety expectations, corrective action, and implications for related HCBS services.
  • Manage implementation, monitoring, and reporting of quality assurance and quality improvement measures for case management and related HCBS case management services, including provider performance, member outcomes, documentation quality, access, continuity of care, and statewide consistency.
  • Support OADS Lifespan Waiver planning and implementation, including provider readiness activities related to Community Resource Coordination and Community Transition Case Management, with attention to conflict-free service delivery, transition planning, staff qualifications, documentation systems, and quality oversight.
  • Collaborate with senior OADS leadership, OMS, licensing staff, providers, advocacy organizations, families, guardians, and community partners to implement policy, provide technical assistance, support provider capacity, address system barriers, and ensure consistent implementation of Adult Community Case Management and emerging Lifespan-related services.

Requirements

  • Four (4) years of progressively responsible experience in social service or mental health/developmental disabilities programs and a Bachelors Degree.
  • At least three (3) years of the required experience must be in a supervisory and/or administrative capacity.
  • Equivalent related work experience may be substituted for education on a year-for-year basis.
  • A Licensed Social Worker (LSW) credential is required.
  • Summarize your supervisory experience managing, coaching, inspiring, and motivating staff to accomplish mission and goals by providing clear direction, accountability, and positive interpersonal influence.
  • Summarize your ability to successfully partner with social service program professionals, MaineCare, licensing staff, providers, advocacy organizations, families, guardians, and community partners to achieve a common goal.
  • Summarize your knowledge of, and experience with, quality assurance initiatives especially related to person-centered planning and thinking, HCBS requirements, provider certification or licensing reviews, and supports to individuals with ID and Autism.
