The Care Team Hospice is seeking an HCHB Integration Application Specialist to support, optimize, and enhance our Homecare Homebase (HCHB) and related application systems across our growing organization. - Fully remote position
- Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM
- Occasional flexibility may be needed to support upgrades, implementations, or organizational initiatives

Key Responsibilities
- Support and optimize Homecare Homebase (HCHB) and related healthcare applications
- Assist with system configuration, implementation, upgrades, and release management
- Troubleshoot complex EMR/EHR issues and identify root-cause solutions
- Support reporting requests and workflow improvements across departments
- Partner with users to improve operational efficiency and application performance
- Provide education, training, and ongoing support to application users
- Participate in merger and acquisition integration efforts as needed
- Collaborate with vendors and internal stakeholders to support system success
Qualifications• Bachelor’s degree preferred
- Two or more years of EMR/EHR application support experience within Home Health and/or Hospice
- Experience supporting healthcare applications such as Homecare Homebase (HCHB), Forcura, Medline, or similar systems required
- Experience with EMR/EHR implementation, configuration, troubleshooting, and user support
- Strong understanding of healthcare workflows, including clinical and back-office operations
- Ability to manage multiple priorities, projects, and deadlines in a fast-paced environment
- Experience with reporting, system upgrades, and process improvement strongly preferred
- Strong communication and problem-solving skills
- Ability to pass a criminal background check
