Role Description The Client Care Coordinator - Home Care plays a critical role in ensuring smooth client support and satisfaction for You're First Care. This is a full-time, remote position focused on coordinating client services, managing communication with clients and care providers, and addressing inquiries to ensure high-quality care. Daily responsibilities include: ¨ Ensure Agency policies and procedures and industry standards and regulations are followed. ¨ Will cover shifts when needed. ¨ Assist with the coordination, evaluation and planning of home care services. ¨ Assist the CCM with administrative activities and optimal use of resources. ¨ Develop, implement and evaluate strategic plans, goals and objectives. ¨ Coordinate staffing activities. Required to cover shifts when staff is unavailable or emergencies. ¨ Coordinate scheduling activities. Approve care charts in EMR system. ¨ Supervise work activities of designated employees. ¨ Develop and instruct employees in the use of practices, procedures and equipment. ¨ Prepare budget reports based on compiled financial and statistical records. ¨ Provide leadership to employees including employee evaluations. ¨ Conduct quality assurance visits. ¨ Liaise with community resources, families and other agencies. ¨ New client intake and assessments. The ideal candidate will serve as the face of the organization, ensuring an excellent client experience.

- Proficient in Microsoft Office Suite and client management systems
- Problem-solving skills and attention to detail
- Previous experience in client care coordination, home health services, or a related field is preferred
- Healthcare Certification is mandatory (CNA/LVN)
