Experienced Remote Social Worker – Medical Case Management and Community Resource Coordination

Job Summary:

We're seeking a licensed Masters Social Worker (MSW) with a passion for delivering exceptional patient care and a commitment to our Heart At Work Behaviors. As a Remote Social Worker, you'll work closely with a registered nurse case manager to assess, plan, and advocate for members' benefit plans and health needs. Your expertise in researching available options and resources for members with financial, transportation, safety, and housing needs will be invaluable in helping us transform our culture and accelerate our ability to innovate and deliver solutions to make health care more personal, convenient, and affordable.

Responsibilities:

- Collaborate with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. - Research available options and resources for members with financial, transportation, safety, and housing needs. - Empower members to make positive lifestyle and medical choices through education and support. - Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health. - Utilize telephonic and electronic exchange to communicate with members and other stakeholders. - Develop and maintain relationships with community resources and organizations to ensure access to necessary services and support.

Essential Qualifications:

- Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) in the state of residence. - 2+ years of experience in medical social work or case management. - Strong communication and interpersonal skills, with the ability to work effectively with diverse populations. - Ability to work independently and as part of a team, with a focus on collaboration and problem-solving. - Strong analytical and problem-solving skills, with the ability to think critically and creatively. - Proficiency in electronic medical records and other relevant software systems.

Preferred Qualifications:

- Experience working in a healthcare setting, with a focus on medical case management and community resource coordination. - Knowledge of community resources and organizations, with the ability to develop and maintain relationships with these stakeholders. - Experience working with diverse populations, including those with complex medical and social needs. - Strong education and training in social work, with a focus on medical social work and case management. - Certification in medical social work or case management, such as the Certified Case Manager (CCM) or the Certified Medical Social Worker (CMSW).
