What To Expect (Job Responsibilities)

• Conduct targeted patient outreach to promote completion of recommended care and preventive screenings
• Schedule appointments and coordinate follow-up care to ensure timely patient services
• Identify and close care gaps by collaborating with clinic teams and referral partners
• Prepare tools and documentation for providers to enhance performance during patient visits
• Maintain organized work queues and participate in team meetings for continuous improvement

What Is Required (Qualifications)

• LVN, Certified Medical Assistant, or equivalent clinical experience required
• 2-4 years of experience in a clinical or physician office setting
• Proficiency with EMR systems, preferably Athena
• Strong organizational and communication skills
• Ability to manage multiple priorities in a fast-paced environment

How To Stand Out (Preferred Qualifications)

• Experience in value-based care, quality improvement, HEDIS, or care coordination workflows
• Familiarity with patient outreach and population health initiatives
• Knowledge of Medicare and Commercial value-based programs
