Experienced Licensed Social Worker – Remote Medical Case Management

As a remote medical case manager, you'll partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. You'll research available options and resources for members with financial, transportation, safety, and housing needs, and work to empower members to make positive lifestyle and medical choices. Responsibilities

As a Licensed Social Worker on our team, you'll be responsible for:

• Partnering with a registered nurse case manager to assess, plan, and advocate for members' benefit plan and/or health needs
• Researching available options and resources for members with financial, transportation, safety, and housing needs
• Empowering members to make positive lifestyle and medical choices
• Collaborating with other clinical staff to assist members with community resources for medical, financial, and behavioral health
• Utilizing a collaborative process to meet members' needs and improve health outcomes

Requirements

* Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) in state of residence

• 2+ years of experience in medical social work or case management
• Masters Degree in Social Work

Preferred Qualifications

* Experience working with diverse populations, including children, adults, and seniors

• Knowledge of community resources and services for medical, financial, and behavioral health
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team

• Base salary range: $46,597.20 - $101,228.40
• Bonus, commission, or short-term incentive program
