- Current PT license and registration in New York.
Experience:
- Proven experience providing adult PT, preferably in a home care setting (though not required).
Communication Skills:
- Strong interpersonal skills to build rapport with patients and collaborate with office staff.
Tech-Savvy: Familiarity with EMR systems is a plus (we provide training). Key Responsibilities: Patient Care:
- Conduct evaluations and create personalized therapy plans to help patients regain mobility and independence.
Flexible Scheduling:
- Coordinate visits directly with patients, choosing when and where you want to work.
Care Planning & Follow-Up:
- Develop and implement care plans, with regular follow-up visits to monitor progress and ensure continuity of care.
Accurate Documentation:
- Record all patient interactions in our easy-to-use, web-based EMR system, ensuring compliance with Medicare and company guidelines.
