Physical Therapist-Home Health (MO)

About the position Provides direct physical therapy to patients in their residence. Receives a physician order for therapy and develops and implements plan of care. Provides direct supervision of qualified physical therapy assistant and evaluates effectiveness of services provided by the assistant. Responsibilities

• Assesses patient functional ability and potential. May perform the initial comprehensive assessment in those instances when the doctor only orders physical therapy services
• Completes PT evaluation to determine potential benefit from therapy and to establish an effective plan of treatment
• Develops and implements medically prescribed PT plan of treatment
• Prepares written plan of treatment for physician’s signature, including clinical procedures and modalities to be used with a specific emphasis on amount, frequency, and duration of treatment
• Understands and interprets medical information from patient records and rest of interdisciplinary team
• Establishes goals and discharge plans
• Interprets and adapts physician orders for the home setting
• Coordinates treatment plan with interdisciplinary team
• Instructs and supervises Home Health support staff, such as HHA, who may carry out portion of patient care
• Instructs and supervises PTA in provided services and evaluates performance and effectiveness and progress of services provided at least every 30 days
• Instructs and supervises patient/caregiver/family member in aspects of PT to be carried out in the home independently
• Accurately and completely documents each visit in a timely manner
• Communicates with physician and interdisciplinary team all appropriate data and changes in patient condition, complications and additional needs
• Orders and / or supervises ordering of specialized durable medical equipment
• Attends and participates in case conference and agency in-service programs
• PT will be required to be on call for weekends and holidays on a rotational basis

Requirements

• Valid state licensure to work as a Physical Therapist
• Current CPR certification
• Ability to work independently
• Knowledge of Microsoft Office or equivalent programs
• Excellent customer service skills
• Excellent problem resolution skills
• Detail oriented and good organizational skills
