LTC Foot Care NP - PT - Jackson, TN

We have been providing services in LTC settings for over 12 years, and we have a very strong reputation among both clinicians and LTCs. Key Responsibilities:
- Provide foot care and/or wound care to residents in long-term care facilities.
- Conduct thorough wound assessments and develop personalized care plans for each patient.
- Perform wound debridement, dressing changes, and other wound care procedures as needed.
- Continuously monitor and assess patients’ progress in wound healing and foot health.
- Educate patients and facility staff on effective foot care best practices.
- Travel to designated nursing homes in your area.
- Communicate regularly with key facility staff, physicians, and healthcare professionals to ensure seamless care delivery and optimize patient outcomes.
- Maintain accurate and up-to-date patient records and documentation.
- Maintain flexibility while seeing patients during weekday working hours.
Qualifications:
- Master’s or Doctoral degree in Nursing from an accredited program.
- Current Nurse Practitioner license in Louisiana.
- Minimum of 2 years of experience in wound care and/or foot care.
- Strong clinical skills in assessment and treatment of wounds and foot conditions.
- Independent, organized, and motivated to provide exceptional patient care.
- Excellent communication and interpersonal skills.
