Experienced Licensed Social Worker – Remote Medical Social Work and Case Management

Job Summary

We're seeking a highly skilled and compassionate Licensed Social Worker to join our remote team. As a Licensed Social Worker, you'll work with a registered nurse case manager to assess, plan, and advocate for members with financial, transportation, safety, and housing needs. This role requires strong communication and interpersonal skills, as well as the ability to work collaboratively with other clinical staff to assist members with community resources for medical, financial, and behavioral health.

Responsibilities

* Research available options and resources for members with financial, transportation, safety, and housing needs

• Partner with registered nurse case managers to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs
• Empower members to make positive lifestyle and medical choices
• Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health
• Conduct telephonic or electronic exchanges with members to assess their needs and develop plans to address them
• Maintain accurate and up-to-date records of member interactions and progress

Requirements

* Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) license in the state of residence

• 2+ years of experience in medical social work or case management
• Masters degree in social work or a related field
• Strong communication and interpersonal skills
• Ability to work collaboratively with other clinical staff
• Ability to maintain accurate and up-to-date records of member interactions and progress

Preferred Qualifications

* Experience working with diverse populations, including children, adults, and older adults

• Knowledge of community resources and services for medical, financial, and behavioral health
• Experience with electronic health records and other software systems
• Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities

• Base salary: $46,597.20 - $101,228.40
