Experienced Remote Social Worker – Medical Case Management and Community Resource Coordination

Job Summary

We're seeking an experienced remote social worker to join our team. As a licensed Masters Social Worker, you'll work with our registered nurse case managers to research available options and resources for members with financial, transportation, safety, and housing needs. You'll empower members to make positive lifestyle and medical choices and work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health.

Responsibilities

* Research available options and resources for members with financial, transportation, safety, and housing needs

• Collaborate with registered nurse case managers to utilize a collaborative process of assessment, planning, and advocacy to meet members' benefit plan and/or health needs
• Empower members to make positive lifestyle and medical choices
• Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health
• Conduct telephonic or electronic exchanges with members to assess their needs and develop plans to meet those needs
• Maintain accurate and up-to-date records of member interactions and outcomes

Requirements

* Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) in the state of residence

• 2+ years of experience in medical social work or case management
• Masters degree in social work or a related field

* Experience working with diverse populations, including children, adults, and families

• Knowledge of community resources and services, including healthcare, financial assistance, and housing
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Familiarity with electronic health records and other healthcare software systems
