Licensed Physical Therapist - Contract (remote)

Abound Health is seeking a Licensed Physical Therapist (PT) to join our Special Consultation Services team in the Charlotte and/or Raleigh, North Carolina areas (Contract). In this role, you will report to the Program Director. As a Physical Therapist working within the Innovations Waiver Program with individuals with intellectual and developmental disabilities, you will assess client needs and develop individualized intervention plans focused on improving mobility, function, and overall quality of life. You will provide consultative services, implement evidence-based therapeutic strategies, and monitor progress—making adjustments as needed to support meaningful outcomes. Through collaboration with caregivers, family members, and interdisciplinary teams, you will empower support systems with the tools and training needed to help clients achieve greater independence and well-being. This is a contract opportunity offering flexibility to create your own schedule and manage your caseload. Telehealth options are available. Key Responsibilities:

  • Conduct assessments, observe clients, and develop individualized intervention plans, including therapeutic exercises, mobility strategies, and recommendations for adaptive equipment or home modifications.
  • Provide training and technical assistance to family members, caregivers, and support staff to effectively implement recommended interventions.
  • Monitor client progress, collect and analyze data, and revise intervention plans to ensure positive outcomes.
  • Participate in interdisciplinary team meetings and collaborate with care coordinators and other stakeholders.
  • Deliver services in client homes, community settings, or via telehealth as appropriate.
  • Ensure compliance with North Carolina Medicaid policies, state regulations, and best practices.
  • Maintain accurate documentation in accordance with NC DHHS guidelines and protect client confidentiality.
  • Report incidents as required and ensure a safe, supportive environment for all clients.

Qualifications:

  • Experience providing physical therapy services and developing intervention plans for individuals with intellectual and developmental disabilities preferred.
  • Strong clinical judgment, analytical thinking, and problem-solving skills.
  • Excellent interpersonal and communication skills to effectively support clients and caregivers.

Education:

  • Must hold a current and valid Physical Therapist (PT) license in the state of North Carolina.
  • Must maintain all required licensure, certifications, and continuing education requirements.

Compensation:

  • For clients receiving additional direct care services, clinicians are paid 70% of the MCO rate.
  • For clients receiving only Specialized Consultative Services (SCS), clinicians are paid 60% of the MCO rate.
  • Flexible scheduling with the ability to build a caseload that fits your availability.
