Job Summary

We're seeking an experienced Licensed Social Worker to join our remote medical case management and advocacy team. As a key member of our team, you'll partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. Your responsibilities will include researching available options and resources for members with financial, transportation, safety, and housing needs. You'll work with a diverse population across the entire lifecycle, utilizing telephonic and electronic communication to empower members to make positive lifestyle and medical choices.

Key Responsibilities

* Collaborate with a registered nurse case manager to assess, plan, and advocate for members' benefit plan and/or health needs

  • Research available options and resources for members with financial, transportation, safety, and housing needs
  • Empower members to make positive lifestyle and medical choices through education and support
  • Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health
  • Utilize telephonic and electronic communication to engage with members and provide support
  • Maintain accurate and up-to-date records of member interactions and outcomes

Essential Qualifications

* Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) in the state of residence

  • 2+ years of experience in medical social work or case management
  • Strong communication and interpersonal skills, with the ability to build rapport with diverse populations
  • Ability to work independently and as part of a team, with a focus on collaboration and coordination
  • Strong problem-solving and critical thinking skills, with the ability to analyze complex situations and develop effective solutions
  • Proficiency in electronic communication and documentation systems

Preferred Qualifications

* Experience working with diverse populations, including children, adults, and older adults

  • Knowledge of community resources and services, including healthcare, social services, and financial assistance programs
  • Experience with case management software and electronic health records
  • Certification in medical social work or case management
