The Specialist’s primary responsibility is to arrange clinically appropriate aftercare plans for all persons served that are discharged from the inpatient detox and rehab units. The Specialist is also responsible for maintaining contact with discharged persons served at specific intervals post discharge to offer encouragement and assistance to remain engaged in treatment suitable to their needs and desires. Serves as a role model in promoting relationships and interactions with persons’ served that support and promote their personal recovery. Arranges both substance abuse and mental health aftercare appointments in locations convenient to the person’s served home and in line with their funding. The Counselor develops the plan with the person served and the Specialist executes it. Works with persons served and counselor to explore and set-up recovery housing or alternative housing post-discharge. Will work with person served to secure funding for recovery housing. The Specialist is an expert in regional substance abuse and mental health programming so they may be a resource to the Counselor as the counselor and the person served develop the aftercare plan. Meets with persons served to complete his/her discharge instructions which details the aftercare plan. The Specialist will always make efforts to make aftercare appointments at providers that the person served had been in active treatment with previously or who referred the person served to us for inpatient care. Subject to the person served approval. Completes follow up calls within 7 days of discharge to confirm attendance at follow-up appointment. Follows-up with persons served who leave treatment AMA after hours to assist with setting up continuing care services. Coordinates referrals to halfway house and specialized residential levels of care; sends referral information, schedules interviews and is the point of contact for providers. Assists with facility transfers as needed. Facilitates psycho-educational groups focusing on aftercare and recovery planning. Collaborates and communicates with treatment team members by actively sharing relevant information with others in a non-judgmental manner, practicing “warm hand-offs”, and developing a shared understanding and respect of the respective roles and responsibilities of team members to ensure collaboration is efficient.
