Job Summary We are seeking a highly skilled and compassionate Registered Nurse - Pediatric Home Health Specialist to provide exceptional care to our pediatric patients in the comfort of their own homes. As a Registered Nurse - Pediatric Home Health Specialist , you will be responsible for conducting home health assessments, developing and implementing care plans, and providing education and support to patients and their families. Key Responsibilities
Conduct comprehensive home health assessments on pediatric patients, including newborns to teenagers Develop and implement individualized care plans, taking into account the unique needs and goals of each patient Provide education and support to patients and their families on various treatments, medications, and medical equipment Monitor patient compliance and adjust care plans as needed Perform various treatments, such as NG or GTube feedings and ostomy care Maintain accurate and detailed records of patient care and progress Collaborate with other healthcare professionals to ensure seamless care coordination Requirements Current and unencumbered state licensure as a Registered Nurse Minimum 1 year of recent clinical nursing practice, preferably in ICU or pediatrics Minimum 1 year of home health experience Demonstrated passion for working in pediatrics Experience with NG or GTube feedings and ostomy care is preferred What We Offer At Thrive Skilled Pediatric Care, we are committed to providing our team members with a supportive and rewarding work environment.
