About the Role

We are seeking an experienced and compassionate Home Health Aide to join our team in Sacramento, CA. As a Home Health Aide, you will play a vital role in coordinating and providing care to our hospice patients, addressing their personal care needs, and promoting their comfort and well-being. You will work collaboratively with a team of healthcare professionals to deliver superior care and will participate in interdisciplinary meetings, training, and development activities. Key Responsibilities:

• Coordinate and provide care to hospice patients, including addressing personal care needs and promoting comfort and well-being
• Calm, soothe, and empathize with patients and families who are experiencing strong emotions
• Participate in interdisciplinary meetings and collaborate with healthcare professionals to deliver superior care
• Assist with observing patients and documenting care
• Collaborate on cases, involvement in training and development, and other organizational activities

Requirements and Qualifications

To be successful in this role, you will need to have:

• Certified Nursing Assistant Certificate issued by the State Department of Public Health (required)
• Certified Home Health Aide issued by the State Department of Public Health (preferred)
• Current BLS/CPR certification
• One year of aide and hospice experience (preferred)
• Possession of a valid California Driver's License, access to a reliable automobile, and minimum car insurance coverage
• Excellent observation, problem-solving, verbal, and written communication skills
• Demonstrated ability to efficiently use a computer, with prior electronic medical record (EMR) experience preferred
• Open to an occasional flexible schedule to work evenings or weekends
• Reliable transportation and comfort traveling throughout the service area
