Job Summary

Key Responsibilities

• Collaborate with healthcare professionals to improve patient outcomes and reduce hospitalizations

Essential Qualifications

• Bachelor's degree in Pharmacy or related field
• Current pharmacist licensure as allowed by the relevant state licensing authority
• Experience in patient care pharmacy programs and clinical pharmacy services
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Proficient in Microsoft Office Suite, including Word, Excel, and PowerPoint
